MDM Cloud Platform Data Model

The Verato MDM Cloud Platform organizes identity data across three core entities—Person, Organization, and Provider—each representing a distinct real-world subject. Within each entity, domain-specific models capture how that identity is expressed and used across different business and healthcare contexts. This structure allows Verato to manage identity consistently while supporting specialized use cases such as patient records, provider credentials, and organizational relationships.

Entity and Domain Structure

Entity and Domain Structure defines how the Verato MDM Cloud Platform models identity by separating real-world subjects into three primary entities—Person, Organization, and Provider—and then layering domain models within each entity to represent how that identity functions in specific business and healthcare contexts. This approach keeps identity consistent at the core while enabling specialized views for use cases like patient management, practitioner credentialing, and facility/organizational relationships.

Person Entity

The Person entity represents an individual identity independent of role or use case.

  • Core Person: Captures foundational identity attributes such as name, demographics, contact details, and household context.

  • Patient: Extends the person identity into a clinical context, supporting healthcare-specific workflows and patient-centric use cases.

Organization Entity

The Organization entity represents non-individual entities such as businesses, institutions, or facilities.

  • Core Organization: Captures organizational identity, structure, and attributes such as names, locations, identifiers, and affiliations.

Provider Entity

The Provider entity represents healthcare-specific identities and is modeled as a specialization of person or organization.

  • Practitioner: Represents individual healthcare professionals, including credentials, licensure, specialties, and practice details.

  • Health Facility: Represents provider organizations and facilities, including organizational attributes, locations, and regulatory identifiers.
